I accept the foundation decision in rendering service activities. I donate abovementioned units regularly on or before 9th of every month. I introduce atleast one person within 30 days from the date of registration. I extend my heartful cooperation for the development of the foundation. I accept to keep my Volunteer-ship on hold, if my donation is not served for 3 consecutive months. I authorize the Foundation to keep record of the abovementioned personal information and to send the relevant foundation messages through SMS and Email.
